The Resident Care Coordinator (RCC) at Savoy Life, a subsidiary of Red Cell Partners, plays a pivotal role in delivering healthcare services to seniors in Chubbuck and Pocatello, Idaho. This position involves close collaboration with a dedicated RN and provider to offer comprehensive care in various settings, including assisted living, independent living, memory care units, and individual homes. Key responsibilities include scheduling patient visits in the electronic health record (EHR) system to optimize the time of RNs and Advanced Practice Providers (APPs), managing inbound communications such as calls, texts, and faxes, and maintaining accurate and organized patient records. The RCC also coordinates patient care to drive positive outcomes, facilitates telehealth visits, and fosters relationships with patients, caregivers, families, and other healthcare providers. Additionally, the role involves educating residents and their families about Savoy's offerings to secure necessary consents.

Candidates must possess a high school diploma or GED, with at least two years of experience in healthcare focusing on care coordination and administrative support. While certification as a Medical Assistant or Patient Care Technician is preferred, individuals with over five years of relevant healthcare experience will also be considered. Experience in geriatric care or within senior living communities is highly desirable. A valid driver's license is required, as the role involves travel between various senior living settings. Proficiency in electronic medical record documentation and adaptability to a dynamic startup environment are essential.
