Aledade is a public benefit corporation founded in 2014, dedicated to empowering independent primary care practices. As the largest network of independent primary care in the country, Aledade helps practices, health centers, and clinics deliver better care to their patients and thrive in value-based care. By creating value-based contracts across various health plans, Aledade aims to transform the traditional fee-for-service model, strengthening continuity of care and aligning incentives to ensure primary care physicians are compensated for keeping patients healthy.
